Description 
The H2H data set, administered by the SA Housing Authority, holds records about people who have experienced homelessness or are at risk of homelessness and who may have been provided with services through contact with specialist homelessness agencies.
Scope
Information about all people in South Australia who are referred to, or seek assistance from, specialist homelessness services (H2H) agencies.

Purpose 
H2H was developed to provide a single, state- wide, case management and information management system. Information from H2H is also utilised for State Reporting, managed by the South Australian Housing Authority, allowing agencies to be provided with additional reports to aid in performance monitoring processes.
As part of their funding agreement, all specialist homelessness agencies are required to use the Homeless2Home (H2H) client and case management system. The H2H benefits both clients and agencies by:
• enabling clients to access assistance through any agency so there is no wrong door
• ensuring clients only tell their story once
• creating easier entry and referral pathway into the support sector for clients
• reducing the reporting burden for agencies
• providing a standardised framework for case management by agencies and;
• Supporting consistent support responses for clients.
